English meaning
That which is the hearing behind the ear, the mind behind the mind, the speech behind speech, the life behind vital breath and the seeing behind the eye: freed from attachment to these faculties, the discerning pass beyond this world and become deathless.
Explanation
An ear behind the ear is not a second physical organ. The expression redirects attention to the ground without which hearing would not occur. Liberation from identification with the faculties and passage beyond death are related interpretive possibilities in the final clause; deathlessness is a spiritual claim, not a promise of bodily permanence.
Context
This answers the opening question by pointing to the enabling ground behind each faculty.
